Consent Forms

Consent Forms enable you to request various consent confirmations, such as consent to store an individual’s data or share their information with third parties for the purposes of a referral.

Once created, consent forms can be signed either in your Advice Centre on a relevant device, or sent to the member to sign digitally.

Permissions

Ensure you have the following permissions to be able create and send consent forms:

  • Advice – Consent Forms
  • Advice – Consent Form Links
  1. Select ‘Consent Forms’ within the Advice module;
  2. Click ‘Create Consent Form’ button and name your form;
  3. Your form will appear in the table - to start building the questions for your form, select the light blue question mark button;
  4. Click ‘Create Question’;
  5. Choose what data input you require for that field:
    1. Information;
    2. Text Input;
    3. Check Box;
  6. Add the question to the ‘Question Text’ box;
  7. Add a form label if you wish – such as ‘Consent to share information' or ‘Name’;
  8. Tick whether or not an answer to the question is ‘Required’ (this is only applicable for ‘Text Input questions');
  9. Enter what order position you want the question to appear in on your form, and click ‘Submit’ to add the question to the form.

Once you have added all of your required questions for the form, you will need to select the blue ‘Finalise’ button to complete its creation and make it available to link to your Advice Cases.

Note: Once finalised, a consent form can no longer be modified. If you wish to make a modification to the form, you will need to create a new form with the change required and remove the old form to ensure it cannot be linked to a case.

  1. Select ‘Consent Form Links’ within the Advice Module;
  2. Click the green ‘Create Form Link’ button;
  3. Select the student you want to link the form to by typing the student name;
  4. Select the form you would like the student to complete;
  5. Click ‘Submit’ to generate a link to be sent to the relevant member to be able to access the form. A unique code will also be generated to provide to the student that they will be required to input in order to complete the form.

You will need to send the link and unique code to the student via email. Ensure that the link and Unique code are sent in separate emails due to Data Protection Regulations.

Once you have created a link, an entry will be created for that member on the main page within the ‘Consent Form Links’ page, listing:

  1. Member Name;
  2. Form Required;
  3. Unique Code;
  4. Whether or not it has been completed (‘Yes’ or ‘No’). Once a member has completed their form, a blue ‘View’ button will appear alongside their form for you to view their completed consent form.

All links will have a red ‘Remove’ button to select alongside them in case they need to be deleted.

Note: If you accidentally remove the wrong form, a new link will need to be created for that member and their consent form re-signed.
