Setting up form options

NB: This step will need to be done before you can use the incident reporting form.

  • On SUMS, go to ‘incident reporting’ and click ‘settings’
  • In settings, against each of the options e.g. injury causes or body parts, click the blue arrow.
  • Click the green e.g. ‘create body part’ button and name the item. The item will be added to the list, and will feed into the drop down on the incident reporting form.

This will allow you to make the system tailored to your Union.
