Submit a Report
There are a number of different ways that an accident, incident or near miss can be submitted on the system.
If you have access to SUMS: * Log into SUMS and go to ‘incident reporting’ and click ‘dashboard’ to get to the incident dashboard * On the incident dashboard, click ‘submit here’ * Complete the information on the incident report form (more details below) * Click the green submit button and you will receive an incident reference number to let you know your submission has been successful
- If you do not have access to the back end of the SUMS system and you are in Lincoln:
- Go to lincolnsu.com/reportanincident
- Complete the information on the incident report form (more details below)
- Click the green submit button and you will receive an incident reference number to let you know your submission has been successful
